(The assumed requirement for this quest is Rank 4. This has not been confirmed as the only requirement.)
To start this quest go to Upper Jeuno and talk to Luto Mewrilah (G-8) After agreeing to help her she will give you a choice of Races. Under each race choice there is a list of names. The options you chose here determine the Race and Name of your NPC companion.

Luto tells you to talk to the people around Jeuno to find information about your NPC. From here there are at least three people you can talk to:

Akta - Ru'lude Gardens (H-10)
Kuah Dakons -
Lower Jeuno (H-8)
Red Ghost - Port Jeuno (G-8)

When you speak to one of these people, they will ask you what kind of hair your NPC has. Answer yes if that is the hairstyle you want. Tell him that's not what your NPC looks like if you don't like that style and talk to a different person.

Example, when chosing my Galka NPC I spook to Red Ghost. He asked if my NPC had a "Large, full beard." I told him no and he suggested I look somewhere else. I then spoke with Kauh Dakons who asked if my NPC had "Spikey hair." I said yes.

After selecting your hairstyle you will be directed to
Upper Jeuno. Head there and find Bheem near the gate (F-5). When you talk to Bheem he will ask you about your NPC's hair color. Based on numerous posts, it seems that saying your NPC has "Bheem's favorite color" hair will get you hair typeA, saying that it's not his favorite color will get you hair typeB.

Return to Luto to complete the quest and get a Silver Ingot. Zone out and talk to her again for the next stage.

First, talk to Luto Mewrilah to get the quest in
Upper Jeuno. She will give you a list, this let's you chose the race and name of your NPC. Make your selection and talk to the people of Jeuno to start tracking your NPC down. My friend and I talked to a Mithra outside of the Merry Minstrel in Lower Jeuno, but Aceofwilds has said that he talked with Red Ghost in Port Jeuno. Either way, the person you talk to will ask you what kind of hair your NPC has, your answer determines their hairstyle.

Next, go to Bheem in
Upper Jeuno (near the gate), he will ask you if the NPC's hair is his favorite color or not. This determines the hair color.

Return to Luto for your Silver Ingot and quest completion. Zone out and talk to her again for the next quest.

Girl in the Looking Glass: Your first fight.
You are now charged with finding a lost little girl; head to back Bheem at the gate house for more info. Bheem tells you that your NPC was heading to Eldime Necropolis, and the little girl was following him. Head into Battalia Downs and go to the (G-8) entrance of Eldime Necropolis. Just inside the zone you'll find a ???. Clicking it spawns an NM skeleton named Namorodo, which you and your NPC must defeat.

Your NPC must survive the fight or you will have to kill the NM again. After defeating the monster, talk to your NPC. They will despawn and tell you they are heading back to Jeuno. Go back to
Upper Jeuno and speak with Luto again to complete this quest. Zone out and back in again to get the third quest in this series.

Mirror, mirror: Fellows united.
When speaking to Luto, your NPC will say the treasure you're looking for is in San d'Oria. Go to Port San d'Oria and find Portaure (H-9) in Cargo Room B. He will tell you that Ghelsba orcs have what you're looking for. Your next stop is the Ghelsba Outpost. Click on the Hut Door (F-10) to enter the battlefield Mirror, mirror.

This is a level 40 cap, 3 person BCNM. Your opponent is the Carrion Dragon (a Dark Dragon type like the Shadow Dragon in Xarcabard). Half way through the battle your NPC will show up to help you finish it off.
Head back to Luto to complete this quest and get your Signal Pearl. This earring will allow you to call your NPC companion to aid you in battle.

Notes:
Namarodo: This NM was very easy with a level 62 WHM, just watch out for Blood Saber as it can drain HP from everyone in the area (posibly bringing it back to full HP). Your party can help you kill Namarodo, but each member must spawn a seperate one. As many as 3 Namarodo's can be spawned at once.

Carrion Dragon: Very easy to solo as a Black Mage. I've heard reports of a level 33 Red Mage soloing this so you needn't worry too much about whether you can beat it. Especially now that 3 people can do the BCNM at once.

Remember: Don't get ahead of yourself. If you haven't been told by an NPC to go somewhere, then you can't go there yet. Talk to eveyone before rushing off. The prospect of getting your NPC is very exciting, I know, but you'll get it much faster if you don't have to backtrack.

Resetting Your NPC: If you are not satisfied with your NPC you have the option of redoing all three quests and getting a new NPC. To do this you must speak to Luto after completing all three quests. The options you're given are "Hear anything new?" and "I want to quit this quest." If you say you want to quit she will ask to confirm that you really want to delete you NPC. Once you've decided to say goodbye to your friend, trade Luto your Signal Pearl to have your memory wiped and go out and do it all again! (Interestingly enough, you get another Silver Ingot when you redo the first quest)

Reobtaining your Signal Pearl: If you accidently throw away your Signal Pearl, you can go to a Rendezvous Point and your NPC will give you another one. (Thank you to Xerolath for this excellent information.)

Clicking on your NPC causes it to leave!! If you accidently click on your NPC be sure to click on it again immediately to keep it from despawning.

You may summon your NPC once every 20 real world hours.

From information gathered so far it seems that you cannot summon your NPC in dungeons or in areas that require quests to access (Tavnazia, Sky, Sea etc.) Another theory is that you can only summon your NPC in areas that a Chocobo can travel to (for example: you can summon in La Theine Plateau, but not
Uleguerand Range.) However, nothing is certain at this time.

Rendezvous Points: You can find these points in each of the main cities. Use these points to summon your NPC to discuss battle tactics and your previous adventures. These points can be found in:
Ru'Lude Gardens (G-10)
Northern San d'Oria (L-10) Just out side the Mog House.
Windurst Waters(K-10) Outside the Mog House. (Thank you Dracumancer)
Port Bastok (J-12) Again, right outside the Mog House. (Thank you Demetres)

You only get one chance for each choice, after you chose, your stuck with it (at least until you finish the quest)

The skeleton NM was extremely easy for a level 62 White Mage.

The BCNM is 40 capped and is 1v1 (until your NPC shows up)

After completing all quests and talking to Luto again you have the options of "Hear anything new?" or "I want to quit this quest" (something along those lines) My guess is that this will allow you to do the quest over and get a different NPC.

You can only summon the NPC once every 20 Earth Time Hours.

CLICKING ON THE NPC KILLS IT! Do NOT click on the NPC. Doing so causes it to say "Is this where we part ways then?" and Despawn. I did this within the first minute and now I have to wait 20 hours to see my friend again

Sorry, I didn't state that correctly. Having the NPC selected and hitting Enter causes the NPC to leave. It does not bring up a menu, it gives you no options, the NPC will simply leave and you will be SoL.

what sucks is once you summon one of these if u zone it dissapears...so if your lvling with it so itll lvl and u get to the point where exp is sucking in an area...{thats too bad} say bye to your npc when you zone

Changing Zones will unsummon the NPC. Yes, it is also true that they cannot be used in all areas.

When you complete the quest, your NPC will also tell you about "Rendezvous Points". These are points in the areas that they specify that allow you to talk to your NPC face to face, view it's level and tactic (ie. "Shield"), and change some combat styles (though limited at this time).

FYI: NPC's are level 30 when you first get them.

NPC's can apparantly be buffed by any spell that doesn't require them to be in party, Protect & Shell & Cures. Party only spells like regen and refresh don't work on the npc, i haven't tried it but from the info above bard songs probably will not affect your npc.

Npc seems to be smart, vokes at the start of the battle(As i was in battila fights didn't last long enough so i don't know if they'll voke mid-fight, but during the bc fight she voked many times fight) and will do various weapon skills. Mine had a sword and did Fast Blade and Burning Blade.

-----DAY 1----- (7/19/2005)
i unlocked the NPC on the first day everyone was able to get on.
at that point.. i changed back to level 16 Samauri and summoned my little buddy for the first time. We only killed about 5 orcs near ghelsba before he said, "That's enough for one day, see you around!" and disapeared... now i wasnt too happy that i had to wait 20 hours after such a short time with my NPC pal.

-----DAY 2----- (7/20/2005)
So i figure ill give my little buddy another try, might as well since im bored after leveling my BLM to 65 earlier in the day. THIS time i switched to my level 5 Dragoon, we had to have killed about 25 things together before he said yet again... "That's enough for one day, see you around!" and disapeared... at this point i was a bit angry.. but i checked him at the rondevou point yet again... and what do i see? hes wearing Black and silver armor(now been identified as: Eisenbrust series)head to toe... omgwtfbbq?... tommorow is a new day with the NPC... will he last longer? will he also hold a new weapon? will more features be unlocked? I personally think there is more than what meets the eye everyone... keep your eyes glued here if you want information on the day-to-day basis.

-----DAY 3----- (
7/21/2005)
Well today was total disaster.. i took him out to rolanberry fields with my 65 BLM thinking everything would turn out great.

About 10 fights later we are fighting a Quadav and BOOM.. it does a special attack on my poor taru.. before i know it hes giving his low health warning... i jump for the Cure III spell and by the time i hit x and start casting hes killed.

I went back to Ru'Lude Gardens and checked him out... it appears he has lost his Hands, Boots, and Pants, but his Body armor still remains.. as well as i can select his job type so not all was a complete loss today. I guess i should have just let him attack and tank and me do nothing but cure him and keep an eye out on his health.

So at the end of today i unlocked a few of his job types, im also going to take the path of HEALER which is what i was hoping for because hes a taru.

-----DAY 4----- (
7/22/2005)
Today went ALOT better, his first day as healer out in rolanberry, what turtlin said was much to be true.. first thing my NPC did was cast protect II and shell on me... at later points in the fight i watched in awe as he used blink and stoneskin as well as aquaveil. At another point i was paraylzed by the ochu and he casted paralyna on me to remove it as well as a few cure III's.

beautfil job square-enix.. beautiful.

i also just dropped my perl and recieved another through my taru with the timer rest. i will take him out again shortly as i have warped back to sandy.

OH and about his equipment.. hes wearing a doublet now... typical for a healer...

-----DAY 5----- (
7/23/2005)
Today went well... i took him to Sanctuary of Zitah, no problems with him as healer, he did a great job.. paralyzed every mob, used dia and even slow. Used a couple Cure III's and always made sure i had protect II and shell.

this time he stayed about an hour.. we were randomly killing level 35-40 gobs with my 65 Blackmage in an attempt to actually LEVEL him to 31 but alas no level was gained STILL..

im still trying to understand why he hasnt leveled ive dont a majority of things people list.

At the Rondevous point... HES WEARING SEER's TUNIC OMFG YESSSSSSS see screenshot above of current/day 5, im getting ready to take him out again i already recieved my perl back after dropping it.. and its lightsday.. that means darks day will soon pass and i can drop my perl again.

-----DAY 6----- (7/23/2005)
Well i took my NPC back out again with my 27 Ninja and fought all Easy Preys and Decent Challenges, no level up... however he did a hell of a job healing and removing poison and paralyze from me... again after about another hour... he said he had to go and left...

checked the rondevous point and his beautiful seer's tunic has now been changed to the Baron outfit. /cry

-----DAY 7----- (
7/24/2005)
Took my NPC to Altep with my blackmage today, no level up or anything.. he actually LEFT me in themiddle of battle this time... "it's getting late... i have to go" and leaves me after only 8 battles... i think theres a certain time limit.

oh well... ill get to use him 2 times tommorow again.. i sorta nerfed today since i droped my perl yesterday and i couldnt get it back... stupid rumors ><

just wanted to throw in that my NPC hit level 31 today.  Here's what i've done so far with it.

Day 1: Took my 51 NIN and fought gobs and quads there and got 0 xp for it before it left. Check NPC and she was wearing chainmail with same scale legs.

Day 2: Took my 52 NIN to
Eastern Altepa Desert and fought some EP beetles and dhalmels for about 10-24xp each. After about 25 minutes, she left again. Checked and noticed she was wearing leather boots...or i like to think they're Leaping Boots^^

Day 3: Took my 52 NIN back to
Eastern Altepa Desert and fought same stuff. She ding'd 31 after about 3 fights. She left again about 25 minutes later. Checked her again and she was wearing different armor...but the big thing is...a new option shows up "Let's talk about jobs." Select it and she says "Which do you recommend?" and i get to choose Shiled style, Attacker Style, or Healer Style" I chose Healer Style^^ so we'll see what happens :)

Also, if you choose Attacker Syle, you get the option for "Signals for weapon skill usage."

I'll update again in 20 hours...

Here are some screenshots:

Day 4: Took my 53 NIN back to Eastern Altepa cept this time i brought a 60 WHM friend who wanted to watch. I summon NPC and engage first monster. The first thing she did was give me Shell II then engaged mob like before and will occassionally debuff with slow, dia, or paralyze. I noticed she did seem to miss more...then again, it might have been my imagination. So i'm letting the monster hit me and as I hit red HP, NPC casts Cure III on me. After fight, I'm in the yellow and she throws another touch up Cure III and then continues to follow me. But the best part was, a beetle silenced me once and she promptly cast Silena on me. My friend and I got a real kick out of that one. I was soo happy. Anyways was having a blast throughout the session. She would Cure me whenever i hit the red and usually would cure again at end of fights. Hardly had to use shihei^^ Other spells she cast were Stoneskin, Aquaveil, Dia, Paralyze, Slow, Protect II, and even Silenced a beetle one >.<

I wish I could see her MP, but when she's low, she'll say she's low. When she's ready to go, she will say so. Session lasted about 30 minutes and got a total xp of about 40xp because of the level 60 with me. Also, she still uses a sword and shield.

After session, went back to Rendezvous Point and saw no change in armor or any new options.

DAY 5: Well, i did the drop Signal Pearl trick found in one of the other threads and took Cupapa out for another stroll. Same as usual, took 53 NIN out to
Eastern Altepa killing EP beetles and dhalmels. On the 8th kill, she hit 32^^. Was hoping to see if she could cast erase...but she left 8 fights later and before i could get any other status ailments. Went back to RP and saw no difference cept she had on Traders Boots and saw no new options. Anyways, can't wait to see what others will report about the Attacker style^^

DAY 6: Took Cupapa out on our 6th date. Same as day 5 except no level. Went back to RP and no new options nor new look. HOWEVER, talked to Luto and got a new CS about how she saw Cupapa at the weapon store and says instead of just giving it to her, I can give it to Luto and she'll make sure Cupapa gets it. Also, instead of giving her the best jeweled weapons, i should give her a beginners weapon. Definately very exciting stuff!! I'll try out various weapons and report back soon.

DAY 7: Same as usual cept she was using a Great Axe this time and it would hit sometimes more than me as 53 NIN...at least it did once. She only used Iron Tempest and Sturmwind once. Also, hit level 33. Besides that, northing new to report.

Day 8: Nothing new to report. Killed 15 monsters at level 33 so i'm thinking that is the limit. If things continue the way they have been, i'm expecting to hit 34 next day and some sort of new option should be unlocked.

Day 9: Same ole same ole. As expected Cupapa gained a level and is now level 34. This occured after the 9th fight which would make it 31 fights from the previous level when she hit 33. She left exactly after the 15 kill once again for at least 4 times in a row now. So I can probably say that if you take your NPC to
Eastern Altepa to kill beetles and dhalmel with a 51-53 NIN by yourself, it will stay out 15 fights. Talked to Luto and got nothing new. Also no new options at RP...although she reverted back to scale mail >.< Heading out for 10th day now.

Day 10: Nothing new to report. Lasted 15 fights again and no new options from either Luto or RP. >.< At least I should hit 35 at next session.

Day 11: Same as day 10 basically. Cupapa hit level 35 after the 10th fight...which makes it 31 fights from when she last leveled to 34. Stayed out for 15 fights. No new options and nothing new from Luto. Hopefully something will come after next date.

Heres some of my personal observations from using the NPC:

All of this happened today while I was leveling my WHM/BLM (33/16).

-In order for your NPC to attack a mob, you HAVE to engage and attack with your weapon.

-If you get aggro, your NPC will not automatically fight the monster who aggro'd you.

-If you cast magic on the mob, like Blind, or Drain, etc, your NPC will not fight until you engage and attack physically.

-As soon as you disengage a mob and put your weapon away, your NPC will do the same and stop fighting.

-While fighting, I got a link at one point, but my NPC only stayed focused on the first monster I physically attacked. I did not try to switch targets because I wanted to finish one monster as quickly as possible before I died XD.

-I did in fact gain the option to change the NPC's job between healer, attacker, and shield types, after summoning it for the third time in three days.

-My NPC decided to leave after I gained around 600 exp. Since I was leveling WHM, this took a while to do, so my NPC lasted around 40 minutes. With that said, based on other people's information, the NPC might stay with you only until you've gotten a certain amount of exp. When mines left he said that "We have met our quota for today." or something along those lines.

Only the following weapons will be accepted after day 5, you may trade any of the following to luto. she will in return give your NPC this weapon.
(she says use a starter for now so later you can give your NPC better weapons)

Uchigatana (Great Katana)
Kunai (Katana)
Bronze Axe (Axe)
Cesti (Hand-to-Hand)
Butterfly Axe (Great-Axe)
Claymore (Great-Sword)
Bronze Knife (Dagger)
Bronze Zaghnal (Scythe)
Ash Staff (Staff)
Harpoon (Polearm)
Xiphos (Sword)
